In order to complete our team, we are looking for a Visual rehabilitation care pathway coordinator Your mission Receive and analyze requests addressed to the visual rehabilitation unit, verify admission criteria, and prioritize interventions Perform a global assessment of medical, functional, social, and environmental needs and correlate them with the priority needs of the beneficiary Direct beneficiaries toward adapted internal or external services Coordinate interventions with stakeholders and ensure follow-up of the support plan Inform, advise, and support beneficiaries and their relatives Facilitate communication between the professionals and partners involved Guarantee the quality and traceability of documentation Ensure the administrative follow-up of the care pathway Contribute to the improvement of practices, tools, and the service's provisions Your assets HES Bachelor or university degree in the health field, or a title deemed equivalent Recognition of the diploma by the Croix-Rouge Suisse is imperative Several years of experience in the health, social, or médico-social field, ideally five years, in Switzerland Experience in visual impairment, disability, or functional rehabilitation desired Additional training in visual rehabilitation or care pathway coordination: an asset Good knowledge of the médico-social network and social insurance in Switzerland Excellent command of French and ease with IT tools We offer you A rewarding activity within a dynamic environment where you can showcase your talents, specific training for our mission, and salary conditions in relation to your skills.
