In this capacity, you will be required to: Provide personalized nursing care at the patients' homes. Assess the needs of beneficiaries and develop adapted care plans. Ensure clinical follow-up and guarantee the continuity of care. Collaborate closely with doctors, relatives, and the various stakeholders in the care network. Actively participate in the promotion of autonomy, comfort, and the well-being of patients. Your profile HES Nurse diploma or equivalent recognized title. For diplomas obtained abroad: CRS recognition is imperative. Confirmed professional experience of at least 2 years in home care. Excellent ability to work autonomously and manage priorities. Strong sense of responsibility, organization, and communication. Strong interpersonal skills and patient orientation. Valid driver's license and personal vehicle indispensable.
