Support to caregivers and orientation: You provide advice and support to relatives, while offering strategic orientations to improve the well-being of clients. Interdisciplinary collaboration: You collaborate effectively within a multidisciplinary team and with an external network for a coordinated approach. Case management: You actively manage client files by acting as a reference person YOUR PROFILE You hold a diploma as a social assistant or a title recognized as equivalent, ideally supplemented by two years of experience in Switzerland. You have solid knowledge of the social insurances of the socio-sanitary system of the canton of Vaud. A certifying training in social insurances would be a plus. Endowed with autonomy, versatility, and ease of adaptation, you have a marked interest in home care as well as social insurances and the helping relationship. You demonstrate a great capacity for listening and communication within a group and with clients. You master administrative procedures with ease and confidence!
