Your versatile tasks are: You are active in direct care and, in complex care situations, you undertake the counseling and guidance of clients, relatives, and employees. You perform needs assessments using the InterRAI instrument (RegioSpitex Limmattal covers the costs for the InterRAI further training). As a case manager, you are responsible for the planning and implementation of the nursing process for your clients. You play an active role in cooperation with the supporting, interdisciplinary network and coordinate the corresponding services for your clients. You bring with you: You have a degree as Dipl. Pflegefachfrau HF / FH as well as ideally corresponding professional experience in this field. You are an open-minded and flexible personality who enjoys approaching people. You bring good communication skills as well as confident handling of mobile nursing documentation (smartphone). You think in networks, work independently, and responsibly.
