In the Memory Care Team of Spitex Zürich, you accompany customers with cognitive impairments in a holistic way - from needs assessment to care and consultation of relatives. You take on case responsibility and design individual solutions that make everyday life easier for those affected and their environment. Your deployments take place during the day - usually between 07:00 and 16:00. Dipl. Nursing Specialist with Case Management - Dementia 60-100% (a) YOUR TASKS You take on case management for people with dementia and control the care process from needs assessment to planning and regular evaluation of the agreed measures. You care for and support our customers in their familiar home, promote their existing resources, and support them in maintaining their independence as long as possible. You advise and accompany relatives, coordinate cooperation with doctors and specialist offices, and are an important contact person for all parties involved. You work within the framework of the Skill & Grade Mix closely with colleagues from different professional groups and ensure careful and seamless care documentation. You network with internal and external specialists, bring your knowledge into the team, and support new employees, learners, and students in their induction.
