Your tasks - You take over the case management for your own client base and coordinate the entire nursing process. - You work closely in an interprofessional setting with our nursing expert, the needs assessment team as well as doctors, therapists and other network partners. - You advise and accompany clients and their relatives in complex care situations. - You ensure professional nursing documentation as well as a high quality of care. - You contribute your professional expertise and actively participate in the further development of our Spitex. What you bring with you - Diploma as Pflegefachperson HF/FH or an equivalent, recognized training in Switzerland - Experience in outpatient care as well as in the coordination of complex care situations - Joy in taking responsibility and actively shaping the nursing process - Independent, structured and solution-oriented way of working - High social competence and joy in interprofessional cooperation - Driver's license category B What you can look forward to Regular working hours Predictable assignments and no split shifts - for a good compatibility of professional and private life.
