Contribute

• Efficient and sustainable discharge planning to increase the satisfaction of patients and relatives as well as to reduce readmissions and rehospitalizations
• Conducting interdisciplinary discussions with patients, relatives, and specialists
• Planning and organizing transfers to rehabilitation centers, spas, nursing homes, or care in the usual environment
• Advising inpatient and outpatient patients as well as their relatives on psychosocial, financial, and social insurance law issues
• General administrative support and coordination for the entire social services team

Bring with you

• Education in Social Work or further training in Case Management
• Ideally some years of professional experience in nursing
• Experience in advising people in stressful life situations
• Strong communication and coordination skills
• Knowledge in the area of social insurance
