Depending on your profile, your mission will consist of: • Evaluating the functional capacities and needs of beneficiaries at home • Developing and implementing an intervention plan that promotes autonomy in daily life activities • Analyzing and adapting the living environment to prevent risks and secure the home • Collaborating closely with the interdisciplinary team and network partners • Participating in health prevention and promotion actions (fall prevention, support for caregivers, etc.) • Ensuring the traceability of interventions and writing reports for partners and insurance companies Location CMS of Rennaz and Montreux Profile • Bachelor's degree in occupational therapy (HES level or equivalent); recognition by the Swiss Red Cross or SEFRI required or in progress for foreign diplomas • Experience in rehabilitation and/or geriatrics, an asset • Strong interest in home care and support for elderly people as well as people with mental health issues • Good analytical and evaluative capacity of living conditions at home to adapt interventions and promote beneficiary autonomy • Excellent relational skills: listening, communication, and negotiation ability • Sense of responsibility and solid professional ethics • Ability to work autonomously while collaborating within a multidisciplinary team • You share our values of transparency, equity, respect, and trust in all your professional interactions • Mastery of computer tools (patient file, planning, quality monitoring) • Driving license essential.
