Care Specialist Outreach Support 60-80%

Stiftung Phönix Zug ( www.phoenix-zug.ch ) provides various care and support services for people with mental health issues at various locations in the Canton of Zug. Over 60 people work for the foundation and its businesses. Phönix Ambulant is one of five businesses of Stiftung Phönix Zug and is aimed at people with a mental impairment who are cared for in their own homes in the Canton of Zug and supported according to their needs. To strengthen our team, we are looking for, as of 1 September 2026 or by arrangement, a Care Specialist (60 - 80%) for outreach support ## What awaits you: ### • Key person work in the support and care of mentally ill people in their homes in the Canton of Zug • Support of those affected in daily life organization according to the objectives set by the affected persons themselves within the framework of a support plan • Regular working hours (Monday to Friday) • Diverse, varied field of work with a lot of creative freedom • Work with the electronic documentation system Nexus Spitex • Interdisciplinary cooperation and coordination with external specialist agencies • Competent, cheerful team of care specialists • Regular team meetings, exchange discussions, and supervisions What we expect: ### • Completed training as a Nursing Specialist HF, as a FaBe, or in Social Pedagogy • At least 2 years of professional experience in the care of mentally ill people • Strong professional and social competence, high level of independence • Reliable coordination and ensuring the flow of information • Resilient, humorous, empathetic, and collegial personality • Professional, exact, and responsible way of working • Competent handling of complex, rapidly changing care situations • Modern IT user skills (MS Office, incl. Outlook).
