YOUR MISSIONS
Central coordination role: As an operational responsible nurse, you coordinate client care, thereby contributing to the quality and efficiency of services.
Integration into the management office: You work in close collaboration with team leaders, planners, and all home care professionals, as well as with the regional care network.
Management of requests and transmissions: You ensure the processing and adequacy of requests, managing urgent and non-urgent transmissions.
Support to field employees: You respond to calls from employees regarding clinical questions, ensuring continuous support in client care.
This position represents a great opportunity for cross-functional collaboration, with active involvement in the CMS d'Yverdon (East, West, Adapted apartments). Please note that working days are fixed and non-negotiable (Wednesday and Friday all day and Thursday afternoon).
YOUR PROFILE
HES diploma in nursing or equivalent, ideally with a specialization in clinical evaluation or analysis.
Deep expertise in the socio-sanitary field with a marked interest in home care maintenance.
Excellent interpersonal skills combined with a keen sense of organization, a strong capacity for adaptation and anticipation, and effective stress management.
Very comfortable with IT tools and competent in telephone communication
