Inpatient Case Manager, fixed-term until 31.12.2026

Your tasks • Co-responsibility for the planning and optimization of the length of stay in close coordination with the responsible physicians, as well as accompanying the inpatient processes of the patients • Coordination and planning of the necessary measures to optimize discharge management in cooperation with all those involved in the treatment and care process, aftercare institutions (rehab, Spitex etc.), patients and, if applicable, their relatives • Early assessment of the post-inpatient care and support needs in coordination with patients and, if applicable, their relatives • Planning and coordination of both inpatient and medically ordered post-inpatient appointments for diagnostics and therapy • Collection of relevant key figures for statistics as well as participation in the creation and implementation of guidelines within the framework of various certifications • Assumption of administrative tasks Your profile • Completed training as a Dipl.
